Our plans’ 2025 service areas, as applicable, include select counties in Alabama, Arizona, Arkansas, Colorado, Florida, Georgia, Hawaii, Illinois, Indiana, Kentucky, Mississippi, Missouri, North Carolina, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, and Washington. 2026 service area depends on CMS contract approval. Any dissemination of Devoted Health plans information for the upcoming plan year prior to October 1 is strictly prohibited. Devoted Health is an HMO and/or PPO plan with a Medicare contract. Our D-SNPs also have contracts with State Medicaid programs. Enrollment in our plans depends on contract renewal.
