Where to Submit Enrollments:

Preferred Method: Via DH Broker Portal
Broker must have an Account on DH Broker Portal. To request an account, send an email to dhmpsales@dhha.org.

Paper Applications:

Via Garity’s Agent Dashboard
Submit and track your enrollments through our secure system.

Via Fax (to Garity): 781-982-1887
Faxing: Please be sure to include a Enrollment Cover Sheet for each application.
Use a separate cover page for each enrollment.

Via SECURE Email (Direct to Denver Health): dhmpsales@dhha.org
DO NOT submit via email if your email is not secure.

All Medicare Advantage enrollments should be submitted electronically. Do not mail the originals.
