Where to Submit Enrollments:
Via CPCT’s Broker Portal for Online Enrollments
Via Garity’s Agent Dashboard
Submit and track your enrollments through our secure system.
Via Fax: 781-982-1887
Faxing: Please be sure to include a Enrollment Cover Sheet for each application.
Use a separate cover page for each enrollment.
All Medicare Advantage and PDP enrollments should be submitted electronically. Do not mail the originals.
